Accident or Near-Miss: If the operator is involved in a forklift accident or a near-miss incident.
New Equipment: If the operator is assigned to operate a different type of forklift.
Workplace Changes: If significant changes occur in the workplace that could affect safe operation, such as new racking, changed traffic patterns, or different surface conditions.
Keeping Records for OSHA Compliance
A certification card is only a summary; the underlying records are what prove compliance. For an operator's certification to hold up during an OSHA inspection, an employer must have documentation showing:
Formal Instruction: Completion of theory training on the specific equipment class.
Practical Training: A documented hands-on training session.
Performance Evaluation: A documented evaluation conducted by a qualified evaluator within the past three years.
